O sucesso inicial da terapia implantar depende da osseointegração e, sua manutenção a longo prazo, relaciona-se à integridade dos tecidos periimplantares. A periimplantite tem sido descrita como uma alteração patológica dos tecidos ao redor dos implantes osseointegrados. A flora bacteriana e o trauma oclusal são considerados os fatores etiológicos principais dessa patologia. Quando diagnosticada prematuramente, essa patologia pode ser reversível. A profundidade de sondagem, o sangramento, a mucosa queratinizada, o fluxo do fluido sulcular e o infiltrado inflamatório servem como parâmetros para avaliar a saúde periimplantar. O tratamento e manutenção da periimplantite incluem a limpeza da área com remoção da placa e do cálculo, orientação e motivação do paciente para higiene oral, destoxificação da superfície implantar, uso de antimicrobianos e terapias ressectivas e regenerativas. A completa descontaminação da superfície do implante parece ser necessária para a resolução da lesão periimplantar. Uma correta intervenção no processo inflamatório torna possível a manutenção dos implantes osseointegrados anteriormente contaminados.

The first sucess of implant therapy depends on Osseo integration and its maintenance for a long time that is related to the integrity of the periimplants tissues. The periimplantit has been described as the pathological change of tissues around the Osseo integration implants. The bacterial flora and occlusive trauma are considered the etiological factors of this pathology. If it is prematurely diagnosed this pathology can be reversed. The depth of a probe, the bleeding tee mucous membrane, the fluid stream of the furrow and the inflammatory infiltrated act as parameter for evaluating the periimplantit. health. The attendance and the periimplantit maintenance includes the cleaness of the area with the removal of the plate and the calculus, orientation and motivation of the patient to do a good oral clearness and the destoxification of the implant surface, with the using of antibiotic and ressective and regeneratives therapies. The full decontamination of the surface of the implant seems to be necessary for resolution of the periimplant lesion. A correct intervention to control and elimination of the inflammatory process becomes possible to obtain the useful life of the osseointegrated implants previous infected.
