Apesar da raspagem e alisamento radicular serem a base da terapia periodontal, é desejado que terapias coadjuvantes sejam também utilizadas, uma vez que nem sempre o objetivo de completa descontaminação das raízes é alcançado. Uma dessas modalidades é a terapia fotodinâmica, onde o efeito bactericida da mesma pode ajudar a melhorar os resultados do tratamento. Assim, o objetivo do presente trabalho foi avaliar clinicamente a efetividade do uso da terapia fotodinâmica como adjunto à raspagem e aplainamento radicular no tratamento não cirúrgico da doença periodontal crônica. Para isso, foram selecionados cinco pacientes, com doença periodontal crônica, apresentando pelo menos dois dentes em quadrantes contralaterais correspondentes com profundidades de bolsas maiores ou igual à 4mm. Os parâmetros clínicos de profundidade de sondagem, nível clínico de inserção e recessão gengival foram avaliados no baseline e após seis meses da terapia, que consistiu em raspagem e aplainamento radicular no grupo controle e, no grupo teste, o mesmo tratamento associado à três sessões de terapia fotodinâmica, utilizando laser de diodo e o azul de metileno a 0,005% como fotossensibilizador. Os resultados apresentados foram diminuição na profundidade à sondagem em 30,14% no grupo controle e 33,49% no grupo teste, e ganho no nível clínico de inserção de 23,69% no grupo controle e 29,68% no grupo teste. Os resultados do presente estudo mostraram que, apesar de ligeira melhora nos resultados, a terapia fotodinâmica não deve ser indicada como uma terapia complementar à raspagem e aplainamento radicular, uma vez que não houve melhoras na redução da profundidade de sondagem e ganho no nível clínico de inserção, considerando-se o protocolo apresentado.

Although the scaling and root planing are the basis for periodontal therapy, it is also desired that adjuvant therapies are used, since the goal of complete decontamination of the root is not always reached. One of this modality is the photodynamic therapy, where the bactericidal effect can help improve treatment outcomes. So, the objective of this study was to evaluate the clinical effectiveness of photodynamic therapy as an adjunct to scaling and root planing in the nonsurgical treatment of chronic periodontitis. For this, it was selected five patients with chronic periodontal disease, presenting at least two teeth in contralateral corresponding quadrants with pockets deep greater than or equal to 4mm. The clinical parameters of probing depth, clinical attachment level and gingival recession were evaluated at baseline and after six months of therapy, which consisted of scaling and root planing in the control group, and the test group received the same treatment associated with three sessions of photodynamic therapy using diode laser and methylene blue 0.005% as photosensitizer. The results presented decrease in probing depth at 30.14% in the control group and 33.49% in the test group, and clinical attachment level gain of 23.69% in the control group and 29.68% in the test group. The results of this study showed that, despite a slight improvement in the results, photodynamic therapy should not be recommended as a complementary therapy to scaling and root planing, since there was no improvement in reducing probing depth and gain in clinical attachment level, considering the presented protocol.
